How to Fill out an HSW/SD Structured Self-report.

(This type of self-report would not be used to self-report study time for the LEED AP Exam. Study time is self-reported under self-designed activity.)

1. Visit WWW.aia.org and click on Education in red;

2. Click on Sign In on the right;

3. Enter AIA member number;

4. Click on Self-Report Form;

5. Click on Create a New Self-Report Form;

6. Enter General Course Information

a. Activity/ Course Title

b. Learning Units/Hours

c. Start date

d. Completion date

7. Under Course Activity Type click Structured Activity Program, fill in Sponsor’s Name and email, and then click Save Form and Continue;

8. Enter 4 things you learned during activity then click Save Form and Continue;

9. Select Core Area, and then click Save Form and Continue;

a. Core areas are topics within the curriculum framework.

10. Select HSW category, and Yes that course qualifies. Then Save Form and Continue;

11. Select SD category and click Yes for course qualifies for SD. Then Save Form and Continue;

12. Select SD topics listed based on category. Then click Save Form and Continue;

13. Select Course Instructional Format and Save Form and Continue;

14. If Course has test/competency element click Yes to competency and what type. Otherwise, click No, and then Save Form and Continue;

15. Verify that all is correct, and then click red button to submit form;

16. Click on Self-Report Online Tracking;

17. Self-Report is listed, and will be reviewed.
